This is a pretty neat doctors surgical kit identified to Dr. W,B Spight . He was a confederate soldier who saw considrable action at some of the bloodiest battles of the Civil War(refer to cover sheet).  After the war he decided to go to medical school and become a surgeon.  He practiced medicine in Mississippi  until his death in 1930. This neat little surgical is very fragile but still retains all it's original tools.  I don't know how rare it is but I've never seen another one.
